Principal Information

Agent (Attorney-in-Fact)

Your chosen agent's full legal name and contact information. Choose someone you trust completely — they will have significant authority over your affairs.

Powers Granted

A specific list of what your agent is authorized to do: manage bank accounts, sign contracts, sell property, make medical decisions, etc. Be precise — vague powers invite disputes.

Limitations & Restrictions

Optional but recommended. Exclude specific powers (e.g., 'Agent may not change my beneficiaries') or add conditions (e.g., 'Effective only upon physician certification of incapacity').

Don't Miss These Critical Clauses
